Part Time LPN/LVN Care Coordinator

The Part Time LPN/LVN Care Coordinator will interface directly with patients, providers, and families within our palliative care program; and manage functionalities such as educating patients by phone and scheduling them for palliative appointments. They'll also be working with clinical files to finalize palliative visits and record data within the systems. The coordinator will allocate their time between phone communication and administrative duties. We are looking for someone with an empathetic ear and strong multitasking skills to ensure patients are getting the care they need. Key responsibilities include: Helping patients understand the value of palliative care Connecting patients with palliative care providers Answering patient questions about palliative care or the program Confirming patient encounters are documented Reviewing follow up with patients Assisting your supervisor by balancing your time between clinical triage and patient calls
