Job Description

Accountable for improving clinical outcomes for empaneled patients, overseeing their medical care, reviewing complex patient cases, developing care plans, and supporting the clinical team in providing evidence-based care. Monitor adverse events and hold clinical retros to identify areas for improvement in protocols, lead development and review of medical protocols related to menopause care, and participate in continuous quality improvement efforts.

Requirements include being licensed to practice as an APP in Pennsylvania, having a minimum of 3 years of menopause experience (including prescribing and managing hormone therapy), and being committed to studying & sitting for the Menopause Society Certified Practitioner exam. Passionate about comprehensive women’s health, with a strong desire to support patients through all stages of midlife care and experience using data to drive patient engagement, activation, and clinical outcomes.

Must have experience working with interdisciplinary teams, eager to thrive in a fast-paced, metric-driven environment, eager to learn using multiple tech platforms and willingness to adapt to new systems. Evening and day shifts available.
