Key Responsibilities:

  • Conduct comprehensive assessments including psychosocial distress and mental health needs.
  • Facilitate advance care planning and serious illness conversations with patients and families.
  • Develop and implement individualized care plans aligned with patient goals.
  • Participate in interdisciplinary rounds and case conferences to coordinate care.

Qualifications:

  • Master's in Social Work from an accredited program.
  • Active, unrestricted LCSW license in applicable state(s).
  • Clinical experience in palliative care strongly required.
  • Strong verbal and written communication skills.
