Key Responsibilities:
- Conduct comprehensive assessments including psychosocial distress and mental health needs.
- Facilitate advance care planning and serious illness conversations with patients and families.
- Develop and implement individualized care plans aligned with patient goals.
- Participate in interdisciplinary rounds and case conferences to coordinate care.
Qualifications:
- Master's in Social Work from an accredited program.
- Active, unrestricted LCSW license in applicable state(s).
- Clinical experience in palliative care strongly required.
- Strong verbal and written communication skills.
