Care Coordinator

This role plays a key part in the remote physiological monitoring (RPM) program by aiding the development and supervision of individualized patient care strategies, offering continuous guidance and support to patients. Responsibilities include monitoring vital signs and care plans through a proprietary platform, collaborating with RN case managers, responding to alerts, and providing patient education via various communication channels. Care Coordinators also facilitate communication between physicians and patients and aid patients in building healthier habits through personalized coaching.
