Responsibilities include monitoring vital signs and care plans through a proprietary platform, collaborating with RN case managers, responding to alerts, and providing patient education via various communication channels. Care Coordinators also facilitate communication between physicians and patients and aid patients in building healthier habits through personalized coaching.
