What You’ll Do:

• Part of the Transitions of Care Team, you assist patients in getting post-hospital care via telephonic and in-person support.
• Outreach to patients discharged from ERs to ensure continuity of care and address social determinants of health.
• Coordinate with facility discharge planners and deliver resources to families in your geographic area.

What You Bring:

• Bachelor's degree required; bilingual (Spanish) and CHW certification strongly preferred.
• 3+ years in a healthcare patient-facing role, ER setting preferred.
• Passion for reimagining pediatric healthcare and commitment to children with special needs.

What We Offer:

• Hourly range $23–28 plus annual bonus and equity purchase program.
