The Behavioral Health Therapist will provide behavioral health services to Eleanor Health patients facing challenges associated with substance use disorder and other mental health diagnoses. The Behavioral Health Therapist will collaborate with the rest of the care team to help each patient achieve their goals. This is a remote position.

Manage a regional behavioral health care team and be responsible for team metrics and program outcomes. Manage a team of cross functional care team members with support of supervisors, while providing oversight and guidance. Partner with talent acquisition to carry out hiring plans and assist with strategic planning for company growth. Analyze programmatic and individual metrics to utilize staff appropriately and identify areas for improvement within team processes and clinical care.

Deliver virtual individual and group counseling sessions that support recovery and promote wellness. Conduct intake assessments to evaluate Member readiness and determine appropriate level of care. Facilitate group and one-on-one counseling sessions using Affect’s evidence-based curriculum. Provide crisis support and coordinate appropriate referrals when needed. Maintain timely, accurate documentation in our electronic medical record (EMR) system.
