We are seeking our first dedicated accounting hire at Form Health, responsible for establishing the accounting foundation, team, processes and systems that will support our next stage of growth. You’ll build the accounting function from the ground up, own the close and reporting processes, oversee internal and external audits, and help scale systems and controls.You will also help to build and manage the team as the company continues to scale.

We are looking for a detail-oriented and collaborative Senior Data Engineer to own and evolve Form Health’s modern data platform. This role is critical to scaling our analytics and operational reporting capabilities as we rapidly grow our patient base, and will ensure we have a robust, reliable, and compliant data foundation. The position will report directly to our Data & AI Lead.

Form Health's Patient Care Coordinators provide comprehensive support for daily clinical operations, and growth of the clinical practice and the company. They provide outstanding experience to patients and increase practice efficiency by working collaboratively with the physician and dietitians. This role is 100% remote.

We seek a candidate with experience with medical scheduling, who is a team player with exceptional interpersonal, critical thinking, and organizational skills. This role is passionate about being part of a health-care team and excited to work in a non-traditional telemedicine practice environment.
