The Senior Strategy Advancement Professional provides data-based strategic direction to identify and address the Primary Care Organization's total cost of care opportunities, involving complex issues requiring in-depth evaluation of economic, clinical, and operational factors. The Senior Professional will support the development, execution, and performance of specialty care initiatives across the organization.

The Senior Specialty Consultative Pharmacist integrates clinical programs to improve overall health outcomes with a focus on Specialty prescription medications filled at CenterWell Specialty Pharmacy. The primary responsibility will be to assess and evaluate the conditions of our members, with a particular emphasis on medication therapy, through telephonic interactions to ensure they achieve and maintain optimal wellness.

Effectively engage members in Care Management via telephonic interactions. Complete necessary assessments and questionnaires with members to identify health risks. Coordinate assistance for members needing doctor’s appointments. Address barriers to various health screenings and connect members with resources to close any gaps in care. Help members retain and access benefits. Service both inbound and outbound calls.
