- Serve on interdisciplinary care team as patient health coach, in partnership with a clinical RN and Pharmacist.
- Empower patients to set their own health and wellness goals (or support them as they strive to stick to a treatment plan from a healthcare practitioner) and offer support as they make progress.
- Provide remote telephonic coaching services at a frequency based on risk, need, and program specific workflows.
