You'd be one of the founding members of our clinical team in new states, shaping how we assess, support, and coordinate care as we launch and grow.
Each 45–60 minute visit is a clinical gateway to months of coordinated geriatric care.

Requirements:

Licensed NP, PA, or MD (must be able to practice in one of the following states: NJ, MD, PA, MA, CA, FL).
Substantial experience in geriatric primary care, transitions of care, or chronic disease management.
