- Conduct thorough assessments to determine the mental health needs of cancer survivors.
- Monitor progress to adjust treatment plans as needed.
Deliver Therapy:
- Administer evidence-based individualized and group survivorship therapy protocols to cancer survivors.
- Support the implementation of an innovative cancer survivorship collaborative care program (CoCM).
Personalized Care Delivery:
- Collaborate closely with other members of the survivorship care team to identify and address patients’ mental health needs.
- Deliver psychoeducation to survivors and their families.
