Job Description
The Care Coordination Manager leads a team of Care Guides to ensure high-quality, person-oriented care for members navigating complex health and social needs. This role is responsible for operational oversight, regulatory compliance, team development, training, and strategic alignment with organizational goals.
Key Responsibilities include team leadership and supervision, program oversight, and ensuring quality and compliance through audits, reviews, and process improvement. Qualifications include a bachelorβs degree in nursing, Social Work, Case Management, or related field, or equivalent experience. Must have a minimum of 3-5 years of experience working in healthcare leading and managing teams and possess exceptional communication, collaboration, and critical thinking skills. Experience working in healthcare IT systems/care management software/Electronic Health Records is needed.
