Senior Director, Care Coordination Operations

Responsible for delivering high-quality, efficient, and compliant care coordination services. This role oversees all aspects of Porter’s Care Coordination services, while leading the Care Guide Team to ensure exceptional member and client outcomes. The Senior Director is responsible for the development, implementation, and continuous improvement of care coordination capabilities to ensure optimal member engagement, outcomes, and client satisfaction. Oversees the Care Guide Team, fostering a culture of accountability, collaboration, and excellence in supporting members through the care continuum. Ensures seamless integration and alignment of all Porter Care Coordination programs with organizational priorities and regulatory requirements. Develops and executes data-driven performance metrics and operational KPIs to monitor effectiveness and drive results.
