Responsibilities:
- Enter into Written Collaborative Practice Agreements.
- Provide supervision and collaborative oversight for CCS Practitioners.
- Review CCS Practitioner Service records.
Qualifications:
- MD or DO with unrestricted license.
- Board certification in family practice, general practice, internal medicine or geriatrics preferred.
- Strong clinical background in evaluation and management of chronic diseases.
Additional Information:
- This is a remote position.
- Minimum time commitment of one (1) hour per week.
- Trainings and orientations arranged and required by CCS, if applicable
