Clinical Assessment Specialist

Client Care:

• Understands and educates clients on the continuum of care.
• Works with the clinical team to engage, educate, and coordinate care with the client.
• Acts as an advocate for all clients when working with external providers.

Assessment and Documentation:

• Completes initial Biopsychosocial Assessments, including all electronic forms.
• Documents all work in the Electronic Health Record.
• Prepares clinical progress notes and other documentation.

Coordination and Collaboration:

• Coordinates client care and crisis intervention.
• Collaborates with external parties, including family members and community providers.
• Ensures effective resolution of client, family, and documentation concerns.
