ABOUT THE ROLE:
- Support diabetes education and management by conducting phone visits for initial member consultations.
- Evaluate diagnoses, medications, and lab results to assess member acuity.
- Support the full clinical organization as a point of contact for clinical escalations.
KEY RESPONSIBILITIES:
- Deliver swift, empathic care to members and assess their clinical competencies.
- Serve as a primary clinical point of contact for practice-wide escalations.
- Adhere to Perry Health policies, procedures, and standard operating procedures.
QUALIFICATIONS:
- Medical Degree (MD or DO) from an accredited institution.
- Valid, unencumbered state medical license required.
- Minimum 3+ years of experience managing patients with diabetes.
