ABOUT THE ROLE:

  • Support diabetes education and management by conducting phone visits for initial member consultations.
  • Evaluate diagnoses, medications, and lab results to assess member acuity.
  • Support the full clinical organization as a point of contact for clinical escalations.

KEY RESPONSIBILITIES:

  • Deliver swift, empathic care to members and assess their clinical competencies.
  • Serve as a primary clinical point of contact for practice-wide escalations.
  • Adhere to Perry Health policies, procedures, and standard operating procedures.

QUALIFICATIONS:

  • Medical Degree (MD or DO) from an accredited institution.
  • Valid, unencumbered state medical license required.
  • Minimum 3+ years of experience managing patients with diabetes.
