Registered Nurse Navigator

Day-to-Day Responsibilities:

• Serve as liaison between patients, providers, and the care team to ensure appropriate support.
• Guide patients through the health system and address barriers to care effectively.

Quality Improvement:

• Identify best practices and support the development of training plans for staff.
• Recommend and implement program or workflow improvements based on data analysis.

Team Collaboration:

• Establish collaborative communication across providers and stakeholders in multidisciplinary settings.
• Participate in meetings and drive coordination of care as a clinical expert and resource.
