Job Description
The Link Health Patient Health Navigators will assist individuals and families in navigating and enrolling in government assistance programs. They will focus on programs like HEAP, Supplemental Nutrition Assistance Program and Lifeline. This is to address social determinants of health by integrating social services, client support, data management, and public assistance knowledge to promote health equity. Volunteers will support patients in navigating benefits applications, engage with the community at partner clinics, enroll patients into programs using Link Health’s dashboard, and complete all compliance documentation. This is an unpaid volunteer position.
