Chronic Care Manager

We are seeking a compassionate and patient-centered Licensed Practical Nurse (LPN) to join our care team as a Chronic Care Manager. This fully remote position plays a key role in supporting patients with chronic conditions by providing proactive, ongoing care coordination and education. Ideal candidates are highly organized, strong communicators, and committed to making a difference in patients’ day-to-day lives. Position is full-time, 40 hours/week, Monday–Friday, with no evenings or weekends. Key responsibilities include conducting patient assessments to develop care plans, collaborating with care teams, monitoring patient health, educating patients, maintaining documentation, and advocating for patient needs.
