Nurse Practitioner

Role Overview:

• You are a Clinical Lead providing longitudinal care for patients with CHF, CKD/ESKD, and other chronic conditions.
• You work in interdisciplinary teams, using advanced analytics and technology to ensure world-class care.
• You reclaim time for meaningful clinical work by reducing administrative burden.

Day to Day:

• Manage a patient panel with accountability for quality, cost, and outcomes.
• Perform patient visits in homes, telehealth, or clinics, collaborating with external providers.
• Document diagnoses for risk adjustment and address care gaps related to HEDIS and preventative care.

Minimum Qualifications:

• Master's degree in Nursing and 2+ years as a Family NP or equivalent with cardiology or nephrology specialization.
• Current state licensure, national board certification, and DEA license eligibility within 90 days.
• Reliable transportation and basic computer skills for travel and telehealth.
