Remote House Calls Nurse Practitioner

Job Summary:

• This role provides virtual clinical oversight, patient assessments, care coordination, and provider support for in-home care.
• Ideal for a nurse practitioner comfortable with telehealth and managing complex chronic conditions remotely.

Responsibilities:

• Conduct telehealth visits and manage chronic conditions like diabetes, hypertension, CHF, and COPD.
• Perform care management needs assessments, follow-ups, and medication reconciliation.
• Coordinate care with primary care providers, specialists, and care management teams.

Qualifications:

• Active NP license in required states and national NP certification (ANCC or AANP).
• Minimum 2+ years of clinical experience, with house calls or home health experience strongly preferred.
• Proficiency with EMR systems and telehealth platforms, plus active DEA registration.
