Role Overview:
- Deliver preventive care to members remotely using Clover's software and approved third-party video apps.
- Complete comprehensive reviews of medical history, physical health, environment, and psycho-social status.
- Document progress notes, prepare workups, and summarize encounters using Clover Assistant and EMR.
Responsibilities:
- Help at-risk members transition from hospitals to home.
- Close gaps in care by ordering labs, writing prescriptions, and coordinating with physicians.
- Perform point of care labs and diagnostic testing as applicable.
Qualifications:
- Based in New Jersey, with active NJ RN or medical license.
- Certified as an Advanced Nurse Practitioner with DEA and CDS registration.
- Minimum 1 year clinical experience and current BLS certification.
