The PCA will advocate for patientβs needs, provide education and guidance, and assist with navigating the healthcare system while removing the complexities of the pharmacy benefit.
This role plays a key part in our remote physiological monitoring (RPM) program by contributing to the development and oversight of individualized patient care plans and providing ongoing guidance and support to our patients. Monitor and review patient vitals and care plans using our proprietary technology platform and collaborate with registered nurse (RN) case managers to support the development and execution of care plans addressing clinical and social needs.
