This position is for a full-time Lead Care Manager to play a critical role in Pair Team's whole-person, interdisciplinary care model, responsible for directly outreaching and engaging with individuals living with Serious Mental Illness/ Substance Use Disorder, experiencing homelessness, and/or those who have high medical needs. The position primarily allows for remote work; however, it includes 1-2 times a month on-site visits in the community.

Responsible for coding denial follow up work queues and appeals processing for FHPG for multi-speciality providers. The role involves denial and appeals claim processing for medical providers, and laboratories services ordered by FHPB, ensuring compliance and generating appeals for denied or underpaid claims.

Full Time Remote Social Worker for our AIM (Advanced Illness Management) Program to join our team covering Augusta, GA. This person will work with patients, their families, PCP, AIM, and other health care professionals to recognize and understand the social/mental stress and/or disorder that magnifies the symptoms related to the serious illness and support/educate patients/families in understanding and executing advance care plan documents that are consistent with their wishes.
