Open Positions
Conduct targeted telephonic outreach to patients and families, educating them on palliative care and completing structured eligibility screenings. Schedule eligible patients for an initial visit with a nurse practitioner and coordinate logistics for a smooth transition to care. Act as a liaison between patients and the care team, maintaining confidentiality and accuracy in documentation per HIPAA standards.
