Certified Medical Assistant

• Answers calls and route all incoming correspondence to appropriate staff.
• Maintains up to date patient health records in the Electronic Medical Record system and Vynca data systems.
• Coordinates transportation for patient medical appointments with contracted transportation companies and payers.

Registered Nurse (RN)

• Manage a panel of seriously ill patients through telemedicine, ensuring visit frequencies align with patient acuity.
• Communicate with patients via phone and video conferencing as outlined in the care plan, providing care within your scope of practice and per provider orders.
• Develop and update care plans based on ongoing assessments, managing medications and coordinating with the Interdisciplinary team and external agencies.

Clinical Lead Care Manager

• Assesses member needs in various areas including physical and mental health.
• Oversees the development of client care plans and goal settings.
• Connects clients to social services and supports that are needed.

Lead Care Manager

The Lead Care Manager serves as the client’s primary point of contact and works with all their providers. The Lead Care Manager manages client cases, coordinates health care benefits, and provides education. The Lead Care Manager collaborates and communicates with client’s caregivers/family support persons, other providers and others in the Care Team.

Registered Nurse

Play a pivotal role in Vynca's Clinical Care Delivery Team as a Registered Nurse. You'll deliver top-notch, evidence-based care to seriously ill patients through telemedicine. This role involves managing patient care, communicating via phone and video, developing care plans, managing medications, coordinating with the interdisciplinary team, and educating patients and caregivers.
