The Medical Director oversees the administration of patient care and provider services, implementing an integrated clinical practice delivery model. They supervise physicians/NPs and collaborate with clinical managers to ensure high clinical delivery standards. This role provides and manages direct health care for a specified patient population, maintaining patient statistics and ensuring compliance with regulations.
As a Senior HR Business Partner at Vynca, you’ll play a key role in shaping a high-performing, mission-driven culture by partnering with leaders across clinical operations, tech, and corporate functions to deliver strategic people solutions that drive employee engagement, retention, and growth. This is a hands-on, strategic role that blends day-to-day HR support with broader organizational impact.
The LCM serves as the client’s primary point of contact and works with all their providers such as doctors, specialists, pharmacists, social services providers, and others to make sure everyone is in agreement about the client’s needs and care. The LCM manages client cases, coordinates health care benefits, provides education and facilitates member access to care in a timely and cost-effective manner.
Direct and oversee operations of the enhanced care management program, including Cal AIM program performance, provider and community collaboration/engagement, and oversight of care management specialists. Develop, implement, supervise, and manage Vynca’s Enhanced Case Management Program. Responsible for administering the ECM program, developing linkage with healthcare plans, outreach, creating clear and easy access and entry into the ECM program, program evaluation, and staff supervision.
The ECM Clinical Manager is responsible for Clinical Oversight and Management of ECM Lead Care Managers to address the client's medical and behavioral health needs and social determinants of health. The ECM Clinical Manager communicates with ECM Lead Care Managers and occasionally with patients to discuss medication management/adherence, ensure follow-up with providers, and guide any crises. They collaborate with the client’s care team to ensure needed services and measure progress.
