The Physician works independently, primarily via video conference, providing palliative care for patients living with serious illness, supported by a team. Responsibilities include performing history and physical exams, ordering and interpreting tests, prescribing treatments, educating patients and families, and ensuring timely referrals to hospice.

The Director of Enhanced Care Management is responsible for directing and overseeing operations of the enhanced care management program, Cal AIM program performance, provider and community collaboration/engagement, oversight of the care management specialists, policy development, process improvement/program enhancements, and developing growth opportunities. They will supervise Clinical Managers, Clinical Consultants and Lead Care Managers.

As a vital member of the Enhanced Care Management team, you will play a crucial role in connecting with prospective clients and guiding them through our services by educating them about our offerings and facilitating their enrollment. The ideal candidate will be personable and professional, adept at reviewing medical charts, and skilled in delivering person-centered care to prioritize tasks effectively and support individuals with complex care requirements.
