- Lead and develop the enrollment team through direct supervision, coaching, and performance management.
- Drive quality, compliance, and scalability of enrollment processes and documentation.

- Complete structured eligibility screenings to identify patients with chronic illness and functional decline.
- Schedule eligible patients for an initial visit with a nurse practitioner and coordinate logistics.
- Conduct outbound calls and intake visits for new clients while logging activities in Salesforce.
- Perform psychosocial assessments and create care plans using evidence-based practices.
- Review medical charts and communicate client needs to the care team for personalized planning.
