As a Lead Care Manager you will serve as the client’s primary point of contact and work with all their providers to make sure everyone is in agreement about the client’s needs and care. The LCM manages client cases, coordinates health care benefits, provides education and facilitates member access to care in a timely and cost-effective manner.

As Program Manager, you will manage a team dedicated to providing longitudinal care management for patients with kidney disease. You will serve as a collaborative and supportive leader responsible for the day-to-day operations of your care management team, focusing on proactive, person-centered care. Your primary duties include managing the performance of team members, providing ongoing training and development, and ensuring adherence to best practices and protocols.

Supports care management activities and the teams assigned to members to ensure services are delivered by the healthcare providers and partners and continuity of care/member satisfaction is achieved. Interacts with members by performing member outreach telephonically or through home-visits and documents the plan for care/services of activities.
