The Director of Enhanced Care Management is responsible for directing and overseeing operations of the enhanced care management program, Cal AIM program performance, provider and community collaboration/engagement, oversight of the care management specialists, policy development, process improvement/program enhancements, and developing growth opportunities. They will supervise Clinical Managers, Clinical Consultants and Lead Care Managers.

As a Director, Post-Acute Care and Kidney Care Excellence, you play a key leadership role in ensuring patients experience seamless, high-quality transitions across the post-acute care continuum, hospital to SNF to home. You will oversee the day-to-day operational delivery of Evergreen’s post-acute care program and lead the successful execution of the PAC workflows across Evergreen’s markets with clear role accountability.

The Clinical Director executes market-level clinical strategy, supports provider performance, and coordinates with cross-functional teams to drive quality, utilization, and cost-of-care outcomes. This role builds relationships with providers and collaborates on how to overcome operational barriers, reduce administrative burden, and optimize clinical strategies for the members' benefit.
