You’ll build and lead our telehealth operations from the ground up, setting the foundation for scalable, compliant, high-impact care delivery. Key responsibilities include designing and launching a scalable telehealth practice that enables community-based organizations to deliver and bill for CHW services. You will build workflows, compliance infrastructure, staffing models and documentation protocols aligned with state and federal requirements.

The Clinical Informatics Specialist supports the implementation, optimization and ongoing maintenance of the organization’s electronic health record (eHR) systems, including Sunrise CareConnect (PointClickCare) and associated clinical applications such as the electronic medication administration record (eMAR). This role collaborates closely with the Clinical Informatics team, care teams and cross-functional partners to design and enhance clinical workflows.
