This level 3 coding position supports the Enterprise Coding Department by coding highly specialized services, requiring advanced coding experience and certification with AAPC or AHIMA. The role involves abstracting information from patient medical records, assigning correct codes, and ensuring compliance with federal and state laws.

Coder III demonstrates proficiency in coding high acuity inpatient accounts and/or coding of technical outpatient accounts to support Revenue Cycle goals for timely billing. Utilizes International Classification of Disease (ICD-10-CM and PCS), Healthcare Common Procedure Coding System (HCPCS) including Current Procedural Terminology (CPT) and other coding references to ensure accurate coding. Examines the complete medical record to accurately determine the diagnoses, procedures, and complications.

Reviews coded health information records to evaluate the quality of staff coding and abstracting, verifying accuracy and appropriateness of assigned diagnostic and procedure codes, as well as other abstracted data, such as discharge disposition. Ensure accurate coding for outpatient, day surgery and inpatient records. Verifies all codes and sequencing for claims according to American Hospital Association (AHA) coding guidelines, CPT Assistant, AHA Coding Clinic and national and local coverage decisions.

Review electronic medical records initiated by clinicians. Verify and code diagnoses, evaluation and management (E/M) services, procedures, and other required codes for record accuracy and completeness. Review and verify the component parts of medical records to ensure accuracy in diagnoses, operations, and special therapeutic procedures.
