Nurse Practitioner (Remote, SC License Required)

Key Responsibilities:

• Provide telephonic outreach and scheduled video/phone visits for recently hospitalized patients, focusing on medication reconciliation and symptom screening.
• Coordinate care across hospital, ED, pharmacies, and community providers to ensure patient continuity.
• Collaborate with nursing team and meet individual and team population health targets.

Compensation & Schedule:

• $78/hour regular rate, $90/hour premium rate for 32-36 hours per week.
• Flexible 4-hour shifts including mornings, afternoons, evenings, and overnight on weekdays and weekends.
• Full-time benefits eligibility at 24+ hours per week.

Requirements:

• Certified Nurse Practitioner in Adult, Family, or Geriatrics with active SC license.
• 5+ years post-graduate clinical experience in Primary Care or Geriatrics.
• Experience with Medicare/Medicaid populations and motivational interviewing skills.
