About the Role:
- You love building relationships with patients based on trust, using motivational communication techniques to drive positive health behavior change.
- You’ll triage vital signs and promote lifestyle changes through wellness calls and patient-centric goals.
- Collaborate with the care team to improve patient outcomes and reduce emergent care.
Responsibilities:
- Manage physiological markers like blood pressure, weight, and blood glucose.
- Perform monthly wellness calls and lead goal-setting with patients.
- Meet team metrics and follow escalation pathways for urgent needs.
What We’re Looking For:
- Unrestricted RN/LPN/LVN license in a compact state and New York.
- 1-3 years of clinical experience with Medicare patients and telephonic care management.
- Ability to work independently and fast learner.
