Registered Nurse, Care Manager

Role Overview:

• Provide real-time clinical support to patients recovering at home after acute care visits.
• Use telehealth platform to navigate recovery safely and comfortably.
• Collaborate with advanced practice providers and operational teams for high-quality care.

Key Responsibilities:

• Triage patient concerns using evidence-based protocols and sound clinical judgment.
• Deliver vital education, reassurance, and escalation support as necessary.
• Document all patient interactions accurately within EMR and communication platforms.

Qualifications & Requirements:

• Hold an active RN license in Compact State and a BSN degree.
• Minimum 3 years post-licensure RN experience with hospital-based nursing preferred.
• Strong EMR proficiency and tech-forward mindset for clinical platforms.

Compensation & Benefits:

• Hourly rate of $44, fully remote role with flexible shifts.
