Engage in collaborative care with Journey Clinical member psychotherapists and work with Journey Clinical care coordinator to streamline your workflow.
Deliver high-quality telehealth care focused on prevention, chronic disease management, and lifestyle interventions to older adults. Provide feedback on the development of clinical protocols, documentation standards, and scalable workflows. Use EMR for documenting patient information. Partner with care coordinators and other clinicians to develop personalized care plans.
