Cardiology Nurse Practitioner

Patient Assessment and Diagnosis:

• Conduct comprehensive telehealth visits including medical history, physical exams, and diagnostic test interpretation.
• Formulate differential diagnoses and develop individualized treatment plans for cardiac conditions.

Disease Management and GDMT:

• Manage chronic cardiovascular conditions remotely with a focus on advanced Heart Failure, hypertension, and arrhythmias.
• Expertly implement and titrate Guideline-Directed Medical Therapy (GDMT) per latest ACC/AHA guidelines.

Collaboration and Patient Education:

• Work closely with cardiologists, PCPs, and care coordinators for comprehensive patient care.
• Educate patients on medication adherence, lifestyle modifications, and disease prevention strategies.

Documentation and Quality Assurance:

• Maintain accurate patient records and ensure compliance with legal and ethical standards.
• Participate in quality improvement initiatives and stay current with cardiology research.
