About the Role:
- We are seeking a part-time Nurse Practitioner to deliver evidence-based, whole-person care to individuals living with chronic GI conditions.
- This role involves collaborating with a multidisciplinary team and providing care via telehealth.
- Ideal for clinicians who enjoy developing meaningful patient relationships over time.
What You'll Do:
- Collaborate with a multidisciplinary team to develop individualized care plans incorporating nutrition, behavioral health, and medication management.
- Evaluate patient symptoms, order lab tests, and provide longitudinal support through telehealth visits and asynchronous communication.
- Utilize evidence-based communication and behavior change strategies to promote patient engagement and self-management.
Who You Are:
- Master of Science in Nursing and national board certification as a Nurse Practitioner.
- Active, unrestricted RN and APRN licenses in at least five U.S. states.
- Experience with chronic disease management, telehealth, and behavior change techniques.
