Responsibilities:
- Contact patients to complete thorough physical, psychosocial, and financial assessments.
- Develop and monitor treatment plans with patients, caregivers, and healthcare providers.
- Advocate for patients to ensure quality care and provide emotional support and guidance.
Qualifications:
- Graduation from an accredited RN program with a current California RN license.
- Minimum five years of medical/surgical or acute care experience, including two in case management.
- Knowledge of medical claims, ICD-10 coding, and community resources; strong interpersonal skills.
