The Care Navigator (part time), serves as the experienced care specialist for Blackbird Health patients and is responsible for engaging and supporting patients who have begun their care journey with a Blackbird Provider. They support families in creating a shared understanding of the precision health care model that Blackbird Provides. They navigate all established patient care and support a positive experience.
