The Home Health/Hospice & Palliative Scheduling Liaison navigates clinical referrals under the direction of the Patient Care Managers to coordinate Field Clinicians patient visits to meet Regulatory specifications from referring providers. This role coordinates staffing/scheduling for all departments ensuring productivity levels for clinicians and Certified Nursing Assistants. Accuracy in scheduling is paramount.
Under the direct supervision of Critical Incident Management RN Supervisor, this position is responsible for Critical Incident Reporting (CIR) activities. They will verify the accuracy of data entered into the Critical Incident Reporting Portal, correct inaccuracies and update the portal appropriately. Complete daily processing of specific types of Critical Incidents on the NM CIR Portal and refers all Critical Incidents for appropriate follow up activities.
Facilitates a team approach, including the Interdisciplinary Care Plan team, to ensure appropriate interventions and cost effective delivery of quality care and services across the continuum. Collaborates with the interdisciplinary care plan team to address care issues and specific member needs whether medical, behavioral, social, community based or long term care services. Coordinates care of individual clients with application to identified populations.
Participant of the Interdisciplinary Care Plan team, to ensure appropriate interventions, cost effective delivery of quality care and services across the continuum. Coordinates care of individual clients using assessment, care planning, implementations, coordination, monitoring and evaluation for cost effective and quality outcomes.
