Responsibilities and Impact:
- Develop trusting long-term patient relationships that truly transform patients’ lives.
- Conduct holistic assessments to identify comorbid conditions and member values.
- Develop care plans in partnership with the interdisciplinary team and caregivers.
Required Skills:
- RN Complex Case Management experience.
- Strong knowledge of interdisciplinary team work.
- Minimum of 5 years of direct clinical experience in acute care.
