The Nurse Practitioner will serve as a vital member of the Clinical Care Delivery Team and will provide evidence-based, high-quality clinical care to an assigned panel of complex and seriously ill patients via telemedicine. Remote care duties performed through HIPAA-compliant hardware and software.

Collaboration between corporate and clinical teams will also be a key aspect of your role in shaping clinical standards and supporting the expansion of care delivery programs.

As a care navigator, you will build relationships with patients, utilizing motivational communication techniques, to help drive positive health behavior change and improved patient outcomes. This program is based around triaging vital signs and using this data to promote positive lifestyle and health behavior changes. You will collaborate with the patient's care team to provide wellness calls with patients to outline patient-centric goals.

As a DaVita Integrated Kidney Care Supportive Care Case Manager (RN) you will support some of our most complex patients, assisting them in navigating a challenging healthcare system. You will identify the medical, social, emotional, and financial needs of your patients with CKD/ ESKD and implement appropriate interventions. You’ll work autonomously and in collaboration with all members of the healthcare team.

As a Director of Nursing | DON | RN, you will be responsible for the administration and management of Nursing Services to residents in accordance with orders of the physicians and the total needs of the residents. You will also be responsible for 24-hour supervision of Nursing Services and directing the Nursing Department to maintain quality standards of care. In the absence of the Administrator, you will assume responsibility for center operations.

This is a remote position with a requirement to live in the Kansas City metro. The job includes reviewing scanned medical records, identifying risk factors, and keeping new patient information updated for future visits. They will pull in outside records into chart from CareEverywhere when available. Strong attention to detail and a passion for improving patient care are necessary.
