Remote Care Navigator (Physical Therapy Assistant - Compact Licensed)

As a Care Navigator, you will be an invaluable early member of our team, ready and eager to make an impact on our company and those we serve. You will support user success by identifying the needs and challenges users have and implementing interventions to guide new thinking and habit change. We’re looking for an entrepreneurial individual to best serve our clients, utilizing methodologies to support your users through their recovery while developing trusting relationships. The ideal candidate is process-focused and eager to make an impact on lives to support musculoskeletal health. You will provide remote monitoring services to help support users as they progress through their home-exercise therapy program Direct and aid users in therapeutic exercise, neuromuscular re-education, and functional training through a tech platform Provide periodic feedback to the evaluating clinician on user progress Maintains necessary records and notes while providing remote monitoring Assist with improving processes and functionality of the platform
