Position Overview:
- Coordinate acute and post-acute stays and discharges for patients
- Build strong relationships with hospital and post-acute providers
- Partner with medical providers, clinical assistants, and care coordinators
Responsibilities:
- Manage patient arrangements for stays and discharges
- Communicate with internal and external stakeholders
- Work to increase coordination of care for a geriatric population
Qualifications:
- Registered Nurse, Licensed Social Worker, or LPN with 4-year degree
- 3-5 years of experience in value-based care or case management
- Valid driver’s license required
