On Call Triage RN (Hospice)

The Hospice On Call Triage Nurses manage the after hours on call administrative operations for assigned hospice branches. This position is for weekends, overnight, and holidays. The ideal candidate will be proficient with Homecare Homebase (HCHB), both PointCare and back office / R2 application. Responsibilities include triaging hospice calls appropriately during the assigned shift and all company holidays, providing an environment that fosters continuous quality assessment performance improvement (QAPI), and maintaining high standards of patient care, integrity, and cost-effectiveness.
