The Admission Coordinator is responsible for regional coordination of patient referrals. They will act as a resource for referral sources, and work closely with branch staff to ensure that adequate coordination and transition occurs to hospice services. Occasional weekends and holidays are required.
Responsibilities include: receiving, documenting, and communicating referrals; gathering and reviewing documentation; ensuring orders for admission are documented; obtaining verbal orders from physicians; obtaining agreement for admission from referred patients; communicating the status of referrals; initiating contact with patients; managing calls from branch clinical office staff; following up with patients in facilities; and coordinating referrals with sales staff.
