Make a warm and welcoming connection with patients as you prepare to collect relevant information and perform various services. Engage in proactive care by conducting thorough health risk assessments, medication reviews, and cognitive screenings. Empower patients with educational information regarding their health and well-being. Work directly with Sprinters to evaluate vital signs, arrange blood draws, and carry out necessary tasks. Provide comprehensive care coordination and management, including preventive care interventions, medication management, and referrals to specialists.
