Conduct high-volume outbound calls to schedule patients for services like mammograms, bone density screenings, and primary care visits. Coordinate care end-to-end and connect patients to community resources addressing social determinants of health barriers. Manage multiple workflows with accurate documentation and collaborate across teams to ensure seamless patient experiences.
