Community Health Worker

The Community Health Worker will educate members on preventative care and provide virtual support for members' health needs, including short-term interventions and collaborative care management in conjunction with our health care partners. They will conduct proactive outreach and maintain engagement with members, especially those with high needs, to help them achieve their health goals, document member interactions and organizing this information efficiently, make necessary referrals to the care team for additional medical or social resources and conduct thorough needs assessments with patients. Furthermore, they will help patients set and reach health goals and manage relationships with healthcare facility providers.
