Senior Care Manager - RN

Job Description:

• Responsible for assessment, care planning, and coordination of services for members.
• Includes ongoing monitoring and communication with PCP and providers.
• Will coach and mentor less experienced Care Managers.

Duties:

• Completes screening and assessment, ensures continuity of care for newly enrolled members.
• Identifies needs, develops goals, and promotes self-management.
• Manages caseload, risk stratification, and facilitates transitions of care.

Qualifications:

• Must be an RN with a Bachelor's degree or have a Master's degree with LCSW, LCPC, or LMFT.
• Experience in home care, long-term care, or MLTC preferred.
• CCM or CCP certification preferred; experience in utilization review or risk management a plus.

Additional Information:

• This is a Temp-to-Perm position.
• Remote position but requires training in local office.
• Supports 24/7 nurse line with caseload of 20-30 calls/day.
