Responsibilities:
- Identify, screen, and coordinate care for patients with chronic conditions in collaboration with primary care providers.
- Perform transitional care management and post-discharge follow-up to ensure timely wellness visits.
- Educate patients on self-management goals and connect them to community resources.
Qualifications:
- Active RN license with 2+ years of population health or care coordination experience.
- Strong computer and EHR skills, and experience with Medicaid patients preferred.
- Ability to travel 25% within the Maryland market.
Benefits:
- Annual base salary range of $75,000 to $85,000 plus 10% annual bonus.
