Role Overview:
- The RN Case Manager works with the clinical department and acts as a liaison with physician practices.
- Advocates for personalized treatment options addressing unique patient care needs.
- Creates and manages the plan of care for patients with chronic or serious conditions.
Key Responsibilities:
- Conduct telephonic clinical outreach to identified patients and identify education needs.
- Collaborate with healthcare teams to coordinate care and help patients stabilize at home.
- Guide patients through the healthcare system and provide community resources.
Qualifications:
- Must be located near Kansas City, MO; Bachelor's in Nursing preferred or Associate's with experience.
- 5 years' RN or RN Case Manager experience with complex care management.
- Minimum 3 years in Med Surg or Home Health; startup experience preferred.
