Role Overview:
- Conduct comprehensive clinical assessments and develop individualized care plans for high-risk members.
- Coordinate transitions of care across emergency departments, hospitals, and skilled nursing stays.
- Collaborate closely with Community Health Workers and interdisciplinary team to address medical, behavioral, and social needs.
Qualifications:
- Active Registered Nurse license in California with 3+ years of nursing experience in care management or related settings.
- Strong experience managing medically complex and high-risk populations.
- Fluency in English and Spanish required.
